Report Cards

Foothill is on a semester grading period.  However, students will receive a progress report at the end of each 6 week period (a triad).  Students will receive a report card at the end of each semester.  Parents are invited to contact the school any time they have concerns.  Below are the dates for the end of the triads and semesters.  Asterisk denotes end of the semester.

Welcome and General Information

Welcome to Foothill Middle School!  Our mission is for all students to successfully learn to the best of their ability in an environment conducive to lifelong intellectual, social, and personal development. 

The FMS vision is that each student will think critically, have a productive work ethic, be intellectually inquisitive, and participate in a collaborative learning environment.  In addition, each student will learn to communicate effectively, be self-motivated, and be a responsible citizen.  Students should be in school daily, on time, with all necessary materials.  Being on time and prepared each day will help maximize the potential for learning and students will develop good work habits for the future.

GENERAL SCHOOL INFORMATION

Emergency Cards/Student Information:  It is extremely important that correct information is on file in the school office for each student.  Please let the school office know when there are changes in address, home telephone, business telephone, or persons to be contacted in an emergency situation.

Health Aide, Illness and/or Injuries:  Any student who becomes ill or injured during the day should report to the health office or main office.  A staff member will contact the parents/guardians to determine if a release from school is recommended.  Any serious injury at school may result in the paramedics being called.  Having family or school medical insurance is highly recommended.

Medications:  Any medications needed by a student during the school day must be kept in the office.  These medications must be brought to school in the original container.  An Azusa Unified School District “Physician’s Recommendations for Medication” form must be completed by the physician and signed by the parent in order to have medication administered at school.  Students may not carry medication (including aspirin, cough drops and cold remedies) at school.  Students may carry asthma inhalers and epipens if it is certified that they are knowledgeable, capable, and responsible enough to use them appropriately.

Nondiscrimination Policy:  It is the policy of the Azusa Unified School District not to discriminate on the basis of sex, race, color, national origin, or handicapping condition in its admission of students to school programs, educational programs, and activities, or the recruitment and employment of personnel.  Students, parents, employees, and community members who feel they have a grievance concerning discrimination should contact Mr. Garry Creel, Equal Opportunity Coordinator, at (626) 858-6168.

Sexual Harassment Policy: It is the policy of the Azusa Unified School District Board of Education that sexual harassment of or by any student shall not be tolerated at school or any school related activity.  Individuals who commit sexual harassment are subject to disciplinary action up to and including suspension and expulsion from the school district.

Renaissance

Renaissance is a nationally recognized program created to recognize students for their academic achievements.  At Foothill we recognize students for perfect attendance, improvements they have made in their grades or in the classroom, as well as many other things.  One of our main forms of recognition is based on students’ Grade Point Average (GPA) from their previous semester.  These are recognized through our Renaissance Cards.

We have four different levels of Renaissance Cards.  The Platinum Card is for students whose unweighted GPA is 4.0.  Gold Card is for students whose unweighted GPA is 3.5 – 3.9.  The Silver Card is for those students whose GPA is between 3.0 and 3.49.  The Bronze Card is for students whose GPAs are between 2.5 and 2.99 or students who have improved their GPA by .5 or greater from one triad to the next.  In order to be eligible for a card you must fit into one of the four categories and you must have no F’s or U’s on your report card.
 
In order to qualify for the end of the year trip to Knott’s Berry Farm a student must have earned four platinum, gold or silver cards or have earned 3 platinum, gold or silver cards along with one bronze card.  Student’s attendance, tardies and citizenship will also be taken into consideration. Rallies will be held to recognize student achievement and accomplishments.
